1. Course Summary

You have now reached the end of the course!

We will use this week as a revision week. You will have time to go over materials from previous weeks, and to revisit forums and activities from previous topics.



2. Care Delivery Action Plans

Have you been submitting Care Delivery Action Plans each week?

If so, you will have quite a list of action points to follow!

If you have not had time to complete the Care Delivery Action Plans, it's not too late!

Why not use this week to look at some of the more important / relevant ones to your own situation?

3. Course Reflection

As we approach the end of Home Based Care - Home Based Education, we would like to invite you to reflect on your participation on this course.

Some people find it useful to record their experience by writing them down on paper. The 'Reflective Model' below might help you to break this task down into manageable component parts.

You could use the above model in the following way:

Awareness: You were aware of your need to do the course. You might like to describe your feelings pre-course and why you sought out training.

Describe the Situation: Here you can describe the course, paying special attention to how you felt while doing the course, for example, how did it feel to participate in Forums where you were sharing sensitive information. Were there any particular 'stand out' moments on the course for you? If so, describe them and what they meant to you.

Analyse Feelings and Knowledge: Here you can try to analyse what you have learned and what it has felt like to come across new meanings / choices / interpretations. Have these caused you to think about doing things differently?  

Evaluate the relevance of knowledge: Does any new knowledge / perspectives gleaned from your participation on the course help in resolving / improving your care situation? If so describe giving some examples.

Identify any learning: Having completed the course are there still gaps in your knowledge? What more do you need to do to improve in practical ways your care situation?

Thank you for enrolling on this course and for participating over the weeks.

Many of you have been very active and visible in your participation. Others have watched and observed from the side lines. Whatever way you have participated on the course, we do hope that you have gained some new insights. And we hope that these new strategies can help you to continue to deliver care for your loved one with dementia.

Remember, these pages will stay open for one year. You are of course most welcome to continue conversations with each other via the Forums. The Delivery Team may drop in here from time to time to see if there are any new postings. But we will not be replying. You can of course drop us an email if you so chose.
